How to Apply for Coverage from AMS in Colorado
There are three ways to apply for coverage from AMS so choose the one that’s easiest for you:
- Apply Online (credit card required for initial month’s premium. Click here to view online quotes and apply online.
- Fax Application (credit card required for initial month’s premium payment): Print the AMS Application and fax the completed application to Mark Erickson at 303-484-3966.
- Phone Application You can apply over the phone after reading and agreeing to the terms, privacy and HIPPA information on the application and then calling 1-866-880-9911. At the beginning of the call give them your agent’s name (Mark Erickson) and his phone number (303-456-7967). Please review the phone application preparation sheet and you’ll be ready to call. You can use a Visa or MasterCard for the initial month’s premium payment and Automatic Check Draft for subsequent monthly billing. AMS has a quarterly direct billing option which has a $15 assessment fee for direct billing.
