after reading and agreeing to the terms, privacy and HIPPA information on the
application
and then calling 1-866-880-9911. At the beginning of the call

You can use a Visa or MasterCard for the initial month's premium payment and Automatic Check Draft for subsequent monthly billing. AMS has a quarterly direct billing option which has a $15 assessment fee for direct billing.
