Tools Adoption Agreement (115Kb pdf file) - The
HSA Tools program includes a checking account, Visa debit card and
access to online banking, limited mutual fund investments and more.

transfer of at least $50 from the personal account of your choice. Also
for a limited time get 6% interest guaranteed for 12 months. Applicants signing up
for HSA Tools must complete page 1.

for a Health Savings Account (HSA) Fundamentals must complete pages 4 and 5 of
