Supplemental Health Insurance from Anthem Blue Cross Blue Shield
Medicare is federal health insurance for people 65 years or older and for those who are eligible due to a disability.
The Social Security Administration recommends that you apply for
Medicare Part's A and B three months before you’d like your benefits to begin. You
will typically receive
Medicare Part's A and B enrollment information in the mail directly from the Social Security
Administration, but if you don't you'll want to call them at
1-800-772-1213.
Medicare Part A is your hospital insurance coverage. Medicare Part B is your medical insurance that cover’s doctor bills and office visits.
While basic Medicare (Part's A and B) covers many of your health costs
please note that they do not cover everything.

Anthem’s Medicare Supplement or "Medigap" plans are available in five standardized plan designs: A, B, F, I and J. All plans provide government-prescribed Basic Benefits, which include:
Basic Benefits
- Hospitalization: Part A coinsurance for the 61st through the 150th day plus 100% coverage for 365 additional days after Medicare hospital benefits end
- Doctor bills and outpatient services: Medicare Part B coinsurance (the 20% of eligible expenses not paid by Medicare) after the Part B deductible ($135 in 2008) is met
- Blood: The first three pints of blood each year

- Basic Benefits as described above
Plan B Benefits
- Basic Benefits as described above
- Part A deductible ($1,024 in 2008)
Plan F Benefits (Most Popular)
- Basic Benefits as described above
- Part A deductible ($1,024 in 2008)
- Skilled nursing care coinsurance for up to 100 days
- Part B deductible ($135 in 2008)
- 100% of Part B excess charges
- 80% of foreign travel emergency costs after $250 deductible for the first 60 days of each trip
Plan I Benefits
- Basic Benefits as described above
- Part A deductible ($1,024 in 2008)
- Skilled nursing care coinsurance for up to 100 days
- 100% of Part B excess charges
- 80% of foreign travel emergency costs after $250 deductible for the first 60 days of each trip
- At-home recovery visits (up to $1,600 a year)
Plan J Benefits
- Basic Benefits as described above
- Part A deductible ($1,024 in 2008)
- Skilled nursing care coinsurance for up to 100 days
- Part B deductible ($135 in 2008)
- 100% of Part B excess charges
- 80% of foreign travel emergency costs after $250 deductible for the first 60 days of each trip
- At-home recovery visits (up to $1,600 a year)
- Preventive medical care (up to $120 a year)
For 2008 Anthem has broken out the prices into two major
categories for people getting ready to turn 65 and older: (1) Anthem Medicare
Plan F Rates , (2) Anthem
Medicare Plans A, B, I & J Rates. If you are disabled
and younger that 65 please call us for those rates. The Anthem
Medicare Supplement Plans for Colorado Outline of Coverage provides
a details on the coverage for each plan option. Please
note the optional consumer Rx discount card is not available if
you've enrolled in Medicare Part D.

ways to apply for
coverage from Anthem so choose the one that is most convenient for
you:
- Fax Application
(credit card required for initial month's premium payment): Print
the Anthem Medicare Supplement application

and fax the completed

- Mail
Application (enclose check for first month's premium
made out to "Anthem
BC/BS") Print the Anthem Medicare Supplement application
and mail the completed

IMPORTANT NOTE FOR APPLICANTS: If you are in the six-month open enrollment period following the
date you reach age 65 and enrolling in Medicare Part B you you
are in the guarantee enrollment period and you can not be turned
down for pre-existing conditions.

section as requested.
